NHS Calderdale CCG: Lead Provider for Integrated Community Beds (Market Test)
NHS Calderdale CCG are looking to work with current providers of health and social care services to develop and implement a flexible nursing bed base which meets both Intermediate Care and Discharge to Assess requirements for step up and step down referrals.
The beds will be referred to as Integrated Community Beds We are looking to appoint a Lead Provider who would identify and work with an existing care home provider to deliver the agreed service specification and flexible criteria for admissions.
The care home should be an existing provider of either residential or nursing beds in the market with a good or outstanding CQC rating.
The care home will have access to beds in a modern environment with the space for therapeutic and rehabilitation to take place with residents.
The Lead Provider will: - Hold the contract for the service and be accountable for service delivery and maintaining quality in line with the standard NHS contract for services - Work with existing system partners including care homes, therapy providers and primary care to develop and implement the service - Provide nursing staff if the home is registered residential The care home will: - Provide modern accommodation and facilities - Provide care workers - Provide nursing staff if the home is registered nursing - Hold CQC registration
