The brief

NHS North West London ICB requires immediate provision of Severe Mental Illness - Health Check, Vaccination, & Screening Support services the borough of Hillingdon.

The service is delivered through a Social Prescriber Link Worker (SPLW) who work's across 6 primary care networks supporting with engagement of SMI patients through outreach work.

The SPLW work's closely with GP practices & other providers to identify appropriate patients specifically those from black & minority ethnic groups who need more intensive support.

The provider will also support with educating practice staff & helping to promote local mental health trainings.

NHS NW London ICB currently has an arrangement in place with The Confederation, Hillingdon CIC to deliver the above services.

This notice is to be transparent about the intention to award a further contract (NHS Standard contract-short form) to The Confederation, Hillingdon CIC from 1st July 2023 until 31st March 2024 (estimated value of £47,603).

Success of the service is linked to close partnership working & integration with the wider pathway of services in Hillingdon.

As such close partnership working & a multi-disciplinary approach will be needed, NHS NW London ICB is of the view that The Confederation, Hillingdon CIC is best placed to deliver the service.

Objectives: -Enable patients to be engaged with in an environment that is more familiar & comfortable to them. -Better understanding of patient's cultural background & ability to be supported using their mother tongue. -Receive a health check which will enable them to improve their health & wellbeing & signpost to other services as appropriate. -Help improve life expectancy as people with SMI die 15-20 years earlier than the general population. -Help increase the uptake of SMI health checks, update of vaccination, screenings -Enables patients who are disengaged or unable to come to the GP practice to be supported & encouraged to receive their Health Check. -Improve trust between SMI patients & their GP practice. -Help reduce health inequalities. -Help to prevent future health conditions & cost to the system.

Key Service Outcomes: -Provide educational briefings. -Provide performance measurement reports on numbers of SMI's. -Provide information on those patients who maybe refusing SMI checks & noting any areas of concern, making safeguarding referrals etc. -Create & co-ordinate all promotional & publicity materials. -Contribute intelligence to key bodies to assist in future service design.
