The brief

Aberdeen City Health & Social Care Partnership wish to engage a social care provider to develop and deliver a Discharge to Assess Test of Change.

This will be run over a maximum period of two years, with built in monitoring and evaluation to ensure that outcomes are being met.

The Test of Change will be delivered by a dedicated team who would work alongside hospital staff in Aberdeen Royal Infirmary to identify individuals who are medically fit to be discharged but require a period of recovery, assessment and enablement in their own home environment.

The level of need of these individuals would normally indicate that they would require a care home environment.

On discharge the service will provide an intensive, targeted period of support and assessment at home, including wrap around 24/7 support where required.

The service will have an enablement focus and include an assessment of a person’s ability to live at home and the level of ongoing support required to enable this.

It is anticipated that following this period the supported person would be able to remain in their own home and avoid alternative accommodation.
